MRCP is a liver function test that evaluates the health of the liver by measuring the level of specific enzymes or proteins in the blood. The liver plays a central role in filtering toxins, producing bile for digestion, metabolising drugs, and synthesising proteins. When liver cells are damaged, they release enzymes into the bloodstream that can be detected and quantified through this test.
What does MRCP measure?
MRCP is a special type of MRI that makes clear pictures of the bile ducts, gallbladder, and pancreatic ducts. It shows fluid-filled tubes and nearby structures without needles or X-rays. This test helps doctors see blockages, stones, narrowing (strictures), inflammation, tumors, or congenital differences. The degree of enzyme elevation correlates roughly with the severity of liver injury: mild elevations (2 to 3 times the upper limit of normal) typically suggest early or reversible changes; elevations above 10 times normal indicate significant acute liver injury. The test processes a standard blood sample through NABL-accredited laboratory equipment, with results typically available on the same day.
What do the results of MRCP mean?
Results are reported in units per litre (U/L) or milligrams per decilitre (mg/dL) alongside the laboratory's reference range. A value within the reference range indicates the liver is functioning normally. Mild elevations (up to 3 times the upper limit of normal) often indicate reversible changes such as fatty liver or medication effect. Moderate to severe elevations (above 5 to 10 times normal) suggest significant liver cell injury and require urgent clinical review, including imaging and additional blood tests.
MRCP Test Preparation
Fasting and dietary instructions:
An overnight fast of 10 to 12 hours is recommended. Eat your last meal the previous evening. On the day of the test, drink plain water freely until your appointment. Avoid alcohol, heavy fatty meals, and vigorous exercise the evening before, as these can temporarily affect several common blood markers.
Medication guidance:
Continue taking all regular prescribed medication unless your doctor has specifically asked you to stop before this test. Inform the lab staff about all medications, supplements, and herbal remedies you are currently taking. Some substances affect specific test values, and this information helps your doctor interpret results accurately.
What to bring:
Bring your prescription, a government-issued ID, and your OPD insurance details. Inform the phlebotomist of any recent illness, active infections, allergy to latex, or history of fainting during blood draws , so appropriate care can be taken during the procedure.
Timing:
Morning appointments are recommended for most blood tests. Biological markers tend to be most stable before meals and physical activity, giving the most consistent, comparable results. Booking an early slot also means you can eat and resume normal activity sooner after the test.

Common triggers include new-onset jaundice, a diagnosis of hepatitis B or C, metabolic syndrome, heavy alcohol use, or elevated liver enzymes found incidentally during a health check-up. It is also ordered at regular intervals to monitor patients already on treatment for liver disease.
Who should get MRCP done?
MRCP is usually ordered as a dedicated imaging study or as part of an abdominal MRI when symptoms like jaundice, unexplained abdominal pain, or abnormal liver tests appear. It helps diagnose bile duct stones, strictures, pancreatitis-related changes, and tumors, and it guides further treatment such as ERCP or surgery. Regular liver enzyme testing is especially valuable for people with fatty liver disease (NAFLD or NASH), patients recovering from hepatitis, those in recovery from heavy alcohol use, and anyone with unexplained prolonged fatigue, nausea, or abdominal discomfort that has not yet been investigated. Early detection of rising enzyme levels allows doctors to intervene before significant, irreversible damage accumulates.
